HOW DO I...
Register With The Practice?

If you wish to register as a patient you need to bring your medical card to reception or complete form GS1. You will be offered a health check with the practice nurse as part of our health promotion programme.

If you have a relative or friend staying with you, they can be seen as a temporary resident. Any foreign visitor may be treated as a private patient and charged a consultation fee.


Please remember to notify the surgery of any change in name, address or telephone number.

HOW DO I...
Make An Appointment?

You may make an appointment to see your doctor by telephoning our receptionist on Godstone (01883) 742387 between 8.30am - 12 noon and 2.00 - 5.00pm. For nurse or routine appointments it is advisable to phone after 10.00am.You may choose to see any of the doctors in the practice, though it is normally better to see one doctor consistently.

For urgent on the day appointments you may not get the doctor of your choice.

If your own doctor is not personally available for any reason, you will be able to see one of the other doctors in the practice. If you cannot keep an appointment, please let us know as soon as possible so that that time can be given to someone else and therefore not waste the doctor's or nurse's time.

If you wish to ask your doctor for advice please ask for a telephone consultation.

Our appointments system offers both 48-hour and advance bookings.

Obtain Assistance Out Of Hours?

When the surgery is closed the local Primary Care Trust is responsible for commissioning this service.

For general medical advice please contact NHS Direct on 0845 4647 or online at www.nhsdirect.nhs.uk

For urgent medical care please phone Thamesdoc on 0300 130 1305.

HOW DO I...
Obtain A Home Visit?

Home visits are for patients who are too ill or infirm to come in to the surgery. Home visits are very time-consuming so, if you can, always come in to the surgery where your doctor has access to our full range of medical facilities.

If you feel that you do need a home visit, please ring the surgery on (01883) 742279 before 10.30am to give us time to organise our day's schedule.
